Skilled Nursing

To include (but not limited to): enteral/parenteral instruction and care, IV antibiotic administration, pain control, wound care, ostomy teaching/care, foley catheters, assessment, patient/family education, diabetic care, venipunctures, bowel and bladder training, teaching ADL’s, diet teaching, medication administration, safety, infection control and other physician ordered services.

Physical Therapy

To include (but not limited to): assessment, therapeutic exercise, gait training, range of motion, ultrasound, hot packs, patient/caregiver teaching, monitoring, safety and infection control.

Occupational Therapy

To include (but not limited to): assessment, teaching, evaluation, independent living/daily living skills (ADL training), muscle re-education, percetual motor training, fine motor coordination, neuro-developmental treatment, sensory treatment, orthotics/splinting, adaptive equipment (fabrication and training), safety and infection control.

Speech Therapy

To include (but not limited to): assessment, patient/caregiver teaching, speech-voice language communication tasks and cuing, voice production activities, speech articulation disorders treatments, dysphagia treatments, non-oral communications, safety, infection control and rehabilitation with aphasia of speech and language skills.

Certified Home Health Aide

To include (but not limited to): bathing, dressing, grooming, caring for hair, nails, or oral hygiene, changing bed linens, shaving, skin and foot care, feeding, assistance with transfers, meal preparation, emptying trash, light housekeeping chores, safety and infection control.

Medical Social Worker

To include (but not limited to): assessment of social and emotional factors related to patient’s illness, counseling for long range planning and decision making, community resource planning, short term therapy, need for care, response to treatments, adjustment to care, financial resources, safety and infection control.
